AN inquiry was launched after a passenger jet took a wrong turn and ended up 4,000 miles from its intended destination.
The Air Asia Airbus was supposed to land at Malaysia but ended up at Melbourne after the pilot entered the wrong coordinates into the internal navigation system.
The incident happened in March but the investigation report has only just been made public.
