MORE people with diabetes will be treated closer to home as part of plans to improve care in County Durham and Darlington.
NHS Durham Dales, Easington and Sedgefield Clinical Commissioning Group says the new care model will greater involve patients in how they are looked after.
It will see more patients treated in the community, rather than at hospital, and follows consultation with patients, carers and families on how best to help people with diabetes stay well and live life to the full.
Dr Winny Jose, clinical lead at the CCG, said, ‘The new model of care will mean GPs and local hospitals working together to give patients care closer to home. The majority of patients will be seen by their GP and practice nurse who can call upon the expertise of Consultants and Diabetic Specialist Nurses.
“Most patients will not see an immediate change to their care, our aim is to start to build diabetes teams in local practices so that patients currently seen in hospital will start to be seen in their local community instead.’
