A GREAT grandmother was the victim of serious neglect due to shortfalls by health agencies and social services, an investigation has found.
Grace Allenby, 88, died two days after being moved into a nursing home where staff were said to be "horrified" by her condition when she arrived.
Before the move, she had been a resident at Grove Lodge Care Home in Middlesbrough, where investigators found she "did not receive an appropriate level of care".
An inquest two years ago found that she died of natural causes, but Middlesbrough Council, which was not directly involved in her care, has now carried out a serious case review which showed she was failed due to "shortfalls" by those who provided her care - including health agencies and social services.
